Our farm is a real, working farm. We ask all of our guests to observe common safety precautions.

All guests must sign a liability waiver. The adult form is here, and the form for minors is below. Guests under the age of 18 who will be visiting the farm unaccompanied by a parent must bring the form below, signed by their parent. Guests under 18 unaccompanied by a parent will not be allowed to visit without a signed liability waiver.

RELEASE
For Minor

I ______________________________________, parent or legal guardian of ________________________________, do release and forever discharge Mike Campbell and Lana Campbell, their heirs and personal representatives, and all firms, corporations, and persons on their behalf liable, from any and all claims, demands, damages, actions, or causes of action arising from or growing out of any and all personal injuries and/or property damage that my child, _________________________, may incur while patronizing Campbell's Farm or Campbell's Farm dba Campbell's Maze Daze or dba Campbell's Fresh Market, Inc., including the mazes, hayride, pumpkin patch, and any other games, activities, entertainment or events.

I HAVE READ THE ABOVE AND FULLY UNDERSTAND ITS PROVISIONS. I ALSO ACKNOWLEDGE THAT I HAVE READ THE FARM RULES AND MAZE RULES AND AGREE TO ABIDE BY SAME AS WELL AS ALL OTHER RULES POSTED ON THE FARM.
