I ___________________________________, do release and forever discharge Mike Campbell and Lana Campbell, their heirs and personal representatives, and all firms, corporations, and persons on their behalf liable, from any and all claims, demands, damages, actions, or causes of action arising from or growing out of any and all personal injuries and/or property damage that I may incur while patronizing Campbell's Farm or Campbell's Farm dba Campbell's Maze Daze or dba Campbell's Fresh Market, Inc., including the mazes, hayride, pumpkin patch, and any other games, activities, entertainment or events.

I HAVE READ THE ABOVE AND FULLY UNDERSTAND ITS PROVISIONS. I ALSO ACKNOWLEDGE THAT I HAVE READ THE FARM RULES AND MAZE RULES AND AGREE TO ABIDE BY SAME AS WELL AS ALL OTHER RULES POSTED ON THE FARM.
